One option missing from your list: lump-sum commissions. Often used in concert with premium memberships or paywalls, but sometimes used disjointly from either. "You pay me [amount], and I create [content] for you." Often used by artists ("anyone can look through my art gallery, but you need to commission me if you want to influence what future content appears therein"), sometimes attempted by bloggers although to much less success from what I've seen.
(In the case of artists, it seems to be pretty common to use commissions sort of analogously to premium memberships, just with a different pay structure; "if you commission a piece, you get the full-resolution PNG, but my public gallery only gets the cut-resolution JPG, or nothing at all if you pay me extra for exclusivity"-type stuff. But I have seen pure instances of the category, occasionally, where it really is purely a matter of deciding what gets produced next.)
